Remember - While gathering documents you may have to login to the account website, many forms are no longer mailed.

Personal Information and Required documents:

  • Name, Address, social security number and date of birth for all named on the return.

  • Copies of Drivers License or State IDs for you and your spouse

  • Proof of Residence for dependents claiming EIC

  • The last two years of income tax returns (New Clients)

  • Dependent Provider info: Name, Address, Tax ID

  • Bank Account and Routing Number for Direct Deposit or Payment (Optional)

  • 1095 A for Proof of Health Insurance IF PURCHASED through the marketplace (Ex: Connect For Health)

  • 1095 B or C if you have Health Insurance through employer

  • Your IP PIN (only if you have asked for one or been a victim of identity theft through the IRS.)

Income documents:

  • Income from jobs: forms W-2 or 1099- NEC for you and your spouse

  • Investment income—various forms 1099 (-INT, -DIV, -B, etc.), K-1s, stock option information

    • Could be from Corporations or Partnerships you own or partly own, Trusts, Estate income etc.

  • Income from state and local income tax refunds and/or unemployment: forms 1099-G

  • Stock or Bond Sales - 1099B

  • Taxable alimony received

  • Business or farming income—profit/loss statement, capital equipment information

  • IRA/pension distributions—forms 1099-R, 8606

  • Sale documents or records for Crypo or Digital Currency

  • Rental property income/expense—profit/Loss statement, rental property suspended loss information

  • Social Security benefits—forms SSA-1099

  • Income from sales of property—original cost and cost of improvements, escrow closing statement, cancelled debt information (form 1099-C)

  • Prior year installment sale information—forms 6252, principal and Interest collected during the year, SSN and address of payer

  • Other miscellaneous income—jury duty, gambling winnings, Medical Savings Account (MSA), scholarships, Foreign Source, Etc.

  • Alimony Income (settled prior to 12-31-2018.)

  • Tips, or any other income you have received.

Other tax documents:

  • IRA contributions

  • Energy credits

  • Student loan interest

  • Medical Savings Account (MSA) or Health Savings Account (HSA) contributions

  • Self-employed health insurance payments

  • Keogh, SEP, SIMPLE and other self-employed pension plans

  • Alimony paid that is tax deductible

  • Educator expenses

  • State and local income taxes paid

  • Real estate taxes paid

  • Gambling expense

  • Personal property taxes—vehicle license fee based on value

  • Home Purchase

  • Estimated tax payment made during the year, prior year refund applied to current year, and any amount paid with an extension to file.

  • Direct deposit information—routing and account numbers

  • Foreign bank account information—location, name of bank, account number, peak value of account during the year

Tax deduction documents:

  • Advance Child Tax Credit payment

  • Child care costs—provider’s name, address, tax id, and amount paid

  • Education costs—forms 1098-T, education expenses

  • Adoption costs—SSN of child, legal, medical, and transportation costs

  • Home mortgage interest and points you paid—Forms 1098

    (This will include forms for HELOC or other second mortgages.)

  • Investment interest expense

  • Charitable donations—cash amounts and value of donated property, miles driven, and out-of-pocket expenses

  • Casualty and theft losses—amount of damage, insurance reimbursements

  • Medical and dental expenses

  • Form 1099-SA to report distributions from an HSA

  • Form 5498-SA to report contributions to an HSA - this information is covered on your W2 if there are no other contributions.

  • Mileage for Medical, Volunteering or Business

  • General Business expenses if you are self employed

  • If you use your home for business—home size, office size, home expenses, office expenses.
